Summary

Under the direction of the Director of Health, within the context of a client and family centered care model and in accordance with the vision and values of Mistawasis Nêhiyawak, this position is responsible for providing enhanced service coordination to children and families in Mistawasis. The Jordan’s Principle Service Coordinator will work with families to help them at many points along the health care continuum. This may include referrals to specialists for their children, understanding treatment and care options, understanding insurance benefits, serving as coach and quarterback of their health care team, working with family members and caregivers, mobilizing resources, managing medical paperwork and finding shelter/housing options. The Jordan’s Principle Service Coordinator will support the restoration, maintenance, and enhancement of social functioning by mobilizing strengths and supporting coping capacities of families.

Duties and Responsibilities (but not limited to)

The Service Coordinator will have three main activity areas:

1. Outreach and Community Engagement

  • Reach out to community members and invite them to access the enhanced service coordination
  • Enhance awareness of existing services and supports for children within Mistawasis
  • Build relationships within the community and collaborate with those providing existing community-based services
  • Work to proactively identify children with unmet needs to facilitate early intervention and timely access to services and resources
  • Enhance opportunities for the success of at-risk clients and families;
  • Locate resources; identify options and available supports (Jordan’s Principle Group and Individual Application Process)
  • Facilitate applications to government/community agencies

2. Intake, Assessment and Coordination

  • Help families navigate the health and social systems in a timely manner
  • Help with referrals for assessments to appropriate professionals, or provide assessments within their scope of practice
  • Build case management capacity through the development of policies, procedures, and protocols related to screening and assessment, service planning, and monitoring and evaluation of services plans and staff training

3. Case Management

  • Provide a collaborative, client driven system to guide children and their families through a comprehensive array of services
  • Support a child and family centered approach to coordinating and connecting services
  • Work towards establishing multi-disciplinary hubs for families when the services they require involves a number of professionals

Qualifications and Experience

  • A degree in social work, nursing, education or related profession and/or 5 years working in First Nations health, social, or education sectors
  • The equivalent combination of education and experience
  • Demonstrated leadership ability in chairing meetings, team building and reporting
  • Vulnerable Sector Check and Criminal Record Check Required
  • A valid driver’s license and reliable vehicle
